Understanding OCD

Obsessive-compulsive disorder (OCD) is a mental health condition characterized by intrusive, unwanted thoughts, images, or urges (obsessions) that cause significant anxiety and distress. To cope with this anxiety, individuals engage in repetitive behaviors or mental acts (compulsions) intended to neutralize the obsession or prevent a feared outcome. These obsessions and compulsions are ego-dystonic, meaning they conflict with a person’s true values, beliefs, and desires, and are experienced as unwanted and distressing. OCD can affect people of all ages and backgrounds. About 1 in 40 adults in the United States have OCD but if diagnosed correctly, effective treatments are available.

First Line Treatments for OCD

is a specialized form of cognitive behavioral therapy and the first-line psychological treatment for OCD due to its strong evidence base. ERP involves working with a specifically trained therapist to identify obsessions, compulsions, and avoidance behaviors, then gradually confronting feared thoughts or situations through planned exposures while resisting compulsive responses. Over time, repeated exposure allows anxiety to naturally decrease without rituals, helping the brain relearn that obsessional fears are false alarms. This process reduces OCD symptoms, builds confidence, and empowers individuals to manage anxiety more effectively in daily life.

Selective Serotonin Reuptake Inhibitors (SSRIs) are the first-line, evidence-based medications for treating OCD, because they are effective for most people with OCD and generally well tolerated. SSRIs include Citalopram, Escitalopram, Fluoxetine, Fluvoxamine, paroxetine and Sertraline; all have comparable effectiveness. Clomipramine may also be used. Treatment of OCD with SSRIs requires significantly higher doses and longer durations of treatment than for depression or anxiety disorders. With any given trial of an SSRI, about 40-60% of patients with OCD will have a clinically significant improvement in OCD symptoms and overall functioning. Medication can be used independently or in conjunction with other treatment options.
